Clearance Process Information
Student-Athletes should visit the CHS Athletic Office to start the Sports Clearance Process. Athletes must get cleared by the Athletic Office for each sport/season and must be cleared before participating in try-outs/practices.

Academic Requirements
All Student Athletes must meet the OSAA and Centennial Academic requirements as listed in the Student Extra Curricular Athletic/Activity Regulations.
Electronic Clearance Forms - Rank One Sport
We have transitioned to include electronic clearing for our student-athletes using the clearing portal RankOneSport.com. On this site, there are three forms that parents and students must complete and sign every year.
Students who were enrolled at Centennial High School at the beginning of the school year can be found in the RankOneSport system. If the student is new to Centennial this year and this is the first sport, please inform the Athletic office so that the student can be manually added to the RankOneSport database.

Sports Physicals
Every student-athlete must provide the Athletic Office with an updated Sports Physical with both sides of the required OSAA document completed, or a completed Health History form (parent/student side of same OSAA document) if the previous year’s sports physical is on file and is still valid. Sports Physicals are valid for a maximum period of two years, unless the physician notes a shorter time frame. The Health History update forms are valid for one year.
- Sports physicals must be valid through the sport season the student-athlete is clearing for (I.E.Fall-through November, Winter-through March, Spring-through May).

Sports Offset Fees and Other Fees/Fines
There is a Sports Participation Offset Fee that must be paid prior to participation in each sport. For the 21.22 school year, sports offset fees are as follows: $130 per athlete per sport, $65 if qualify for reduced meal benefits, $0 if qualify for free meal benefits. Families must apply annually for free/reduced meal benefits beginning each July 1. Applications are online at this link: On-line Meal Benefits Application. Student-athletes must also pay all outstanding fees/fines owing to CHS prior to participation.
New Students
If your student is planning to transfer from another school or has already transferred from another school this year, please come to the Athletic Office for additional information regarding the requirements for athletic participation. Stop by, or call 503-762-6137 to schedule an appointment.
Students Attending Middle College, Web Academy, Private School, or Home School
If your student resides in Centennial School District, but will attend classes at a middle college, web academy, private school, or will be home-schooled, please come to the Athletic Office for additional information regarding the requirements for athletic participation. Medical Insurance is Required
All Student athletes must have medical insurance. Due to recent expansions of Oregon’s medicaid program, most Oregon youth (under 19) qualify for the Oregon Health Plan, regardless of their immigration status. For more information about eligibility requirements, visit the state’s OregonEligibility webpage. Families needing support applying for Oregon Health Plan coverage can contact the Multnomah County Student Health Center at Centennial High School: 503-988-5488.
